A patient tells me they are exhausted, their hair is thinning, and they get breathless climbing stairs they used to manage easily. Their hemoglobin comes back normal, and they are told nothing is wrong. Often, something is — it just has not reached the stage hemoglobin catches yet.
Iron deficiency happens in stages, and hemoglobin is the last one
Your body has iron reserves, stored primarily as ferritin, before it ever touches the iron actively being used to make hemoglobin. Depletion happens in order: stores fall first, then iron available for use falls, and only once both are substantially depleted does hemoglobin production actually suffer enough to show up as anemia on a standard blood count. This means a normal hemoglobin can coexist with genuinely low iron stores for months or years.
Why ferritin is the number that catches it early
Ferritin reflects your stored iron directly. Low ferritin with normal hemoglobin is a real, symptomatic state for many people — not a lab curiosity. Fatigue, hair thinning or shedding, restless legs, brittle nails, and reduced exercise tolerance can all appear at this stage, before anemia sets in, and are frequently dismissed because "the blood count is fine."
Why this stage gets missed so often
Ferritin is not part of a standard complete blood count — it has to be specifically requested. Many routine checkups never include it unless a doctor is specifically thinking about iron status, which means the early, symptomatic stage of deficiency is invisible unless someone asks the right question.
Who is at genuine risk
Menstruating women, given regular blood loss — this is the single largest risk group, and deficiency here is common enough that it should be actively screened for rather than waited on. Vegetarians and vegans, since plant-based (non-heme) iron is absorbed considerably less efficiently than iron from meat. Frequent blood donors. Endurance athletes, through a combination of increased iron turnover and, in runners specifically, some direct loss through the gut. Anyone with unexplained fatigue lasting more than a few weeks deserves this checked, regardless of which group they fall into.
An important caution in the other direction
Ferritin is also an inflammation marker, and it can appear artificially normal or elevated during infection or inflammation even when true iron stores are low, which is why interpretation sometimes needs additional markers alongside it, not ferritin in isolation.
What actually helps absorption if supplementing
Vitamin C alongside an iron-rich meal meaningfully improves absorption of plant-based iron. Tea and coffee, taken close to an iron-containing meal, meaningfully reduce absorption — spacing them an hour or two apart from meals or supplements makes a real difference. Supplementing without a confirmed low ferritin is not advisable; iron overload is a real and different problem, and self-supplementing based on symptoms alone risks masking or worsening an unrelated issue.
What to actually ask for
If you have persistent fatigue, hair shedding, or restless legs and a normal hemoglobin, specifically ask for a ferritin test rather than assuming the normal blood count has ruled out iron as a cause.
