Nearly every routine checkup includes a complete blood count, and nearly every patient receiving one reads it as a wall of unfamiliar abbreviations with arrows next to a few of them. Here is what the numbers that most often carry arrows actually mean.
Hemoglobin is usually the first thing to check
Hemoglobin carries oxygen in your red blood cells, and low hemoglobin is anemia — a finding, not a diagnosis in itself. The number tells you anemia is present; it does not tell you why. That distinction matters because the causes range from simple iron deficiency to chronic disease to, rarely, something more serious, and the workup differs entirely depending on the cause.
MCV tells you what kind of anemia you are dealing with
Mean corpuscular volume describes the average size of your red blood cells, and it is one of the more useful numbers on the whole panel for narrowing down a cause. Small cells (low MCV) point toward iron deficiency or thalassemia. Large cells (high MCV) point toward vitamin B12 or folate deficiency, or sometimes alcohol use or thyroid disease. Normal-sized cells with anemia present suggest a different set of causes again, including chronic disease or recent blood loss. This single number meaningfully directs where I look next.
White blood cells and what an elevated count actually signals
An elevated white cell count most commonly reflects your body responding to infection or inflammation — it is a sign of your immune system working, not a diagnosis of anything specific on its own. The differential, which breaks the white cells down by type, adds more information: a rise dominated by neutrophils points more toward bacterial infection, while a rise dominated by lymphocytes points more toward viral infection. A significantly low white cell count is a different concern and, depending on how low, sometimes warrants urgent evaluation, since it can mean reduced ability to fight infection.
Platelets and why both directions matter
Platelets are involved in clotting. Low platelets can mean increased bleeding risk; the causes range from viral infections, which usually resolve on their own, to more serious bone marrow or autoimmune conditions. High platelets are usually reactive — a response to inflammation, infection, or iron deficiency — though persistently very high counts warrant further evaluation.
What one abnormal number, alone, usually means
A single value slightly outside the reference range, with everything else normal and no symptoms, is frequently a lab-to-lab or day-to-day variation rather than a meaningful finding. What changes my level of concern is a pattern: multiple abnormal values together, an abnormal value alongside symptoms, or a value that has clearly changed compared with a prior test.
What to actually do with your CBC report
Look at what has an arrow next to it, note the specific number rather than just "high" or "low," and if you have an old report, compare rather than just reading the current one in isolation. Bring both to your next appointment if something looks off — the comparison is often more informative than either result alone.
